  2. Paperwork: We'll send new-patient forms through the patient portal so you can complete them at home.
  3. Arrive: 10 minutes early for your first visit. Bring photo ID, insurance card, and a list of current medications.
  4. Visit: Most appointments take 20–40 minutes with Dr. Hudgens or one of her providers.
  5. Follow-up: Any prescriptions, lab results, or follow-up scheduling will be confirmed before you leave.

Forms

New-patient paperwork.

New-patient forms are completed electronically through our patient portal. Once you schedule, you'll receive an email or text invitation to log in and complete your registration before your visit.

Common questions

Quick answers.

How do I become a new patient?

Call, text, or use the appointment request form. We'll reach out to schedule and send your paperwork.

What insurance do you accept?

We accept most major plans. Text or call the office and we'll verify yours before your visit.

Where do I park?

Free parking is available throughout The Grove at Isleworth. Our suite is 280, on the second floor of 4750 The Grove Drive. The elevators to the office are through the glass doors behind Jeremiah's Italian Ice.

What should I bring to my appointment?

Photo ID, your insurance card, a list of current medications and allergies, and any previous dermatology records you'd like us to review.

Can I refill a prescription without an appointment?

Prescription refills are available to established patients who have been seen within the past year. Request a refill through the patient portal. If it's been more than a year since your last visit, or if you need a new prescription or a significant dose change, we'll need to see you first.
